What Are Obsessive Compulsive and Related Disorders?

Obsessive-compulsive and related disorders are characterized by repetitive thoughts, urges, or behaviors that cause significant distress and interfere with daily functioning. While obsessive compulsive disorder (OCD) is the most well-known condition in this category, the group also includes body dysmorphic disorder, trichotillomania (hair-pulling disorder), and excoriation (skin-picking) disorder. These conditions share common features, including repetitive thoughts or urges, compulsive behaviors aimed at reducing distress, and difficulty controlling these patterns despite awareness that they may be excessive or irrational.

How Common Are These Disorders?

Obsessive-compulsive and related disorders affect approximately 1-3% of the population. These disorders typically begin in childhood, adolescence, or early adulthood and can persist throughout life if left untreated. They affect people across all ages, genders, and backgrounds. Many people with one obsessive-compulsive and related disorder may also experience other anxiety disorders, depression, or related conditions.

What Causes Obsessive Compulsive and Related Disorders?

Multiple factors contribute to the development of these disorders, including genetic predisposition, neurobiological differences in brain structure and function (particularly involving serotonin and areas of the brain related to habit formation and anxiety), early life experiences, learned patterns of coping with anxiety or distress, personality factors such as perfectionism or need for control, and triggering life stressors or traumatic events.

How Can Treatment Be Helpful?

Cognitive-behavioral therapy, specifically including exposure and response prevention (ERP), has been shown to be highly effective for obsessive-compulsive and related disorders. Treatment helps you understand the cycle that maintains these behaviors, learn to tolerate anxiety and uncertainty without engaging in compulsive behaviors, gradually face feared situations or stimuli, develop healthier coping strategies, and challenge underlying beliefs that fuel these patterns.

Both during therapy sessions and through self-help homework assignments, you will learn specific techniques tailored to your condition. Treatment may include gradual exposure to triggers, response prevention or habit reversal, cognitive restructuring to address distorted thoughts, mindfulness and acceptance strategies, and stress management techniques.

How Effective Is Treatment?

Research consistently shows that cognitive-behavioral therapy, particularly ERP, is highly effective for obsessive-compulsive and related disorders. While treatment requires commitment and can initially increase anxiety, most people find that their symptoms become much more manageable over time.
